Health care proxy program

Name someone you trust to speak for your care.

Right to Care lets an adult Iloilo City resident designate another unrelated adult resident as their health care proxy. The agent can make medical decisions on your behalf only if you lose the capacity to make them yourself — and you can revoke it at any time.

How it works

  1. Apply online

Submit your details and your chosen agent's details. Both of you must be at least 21 and residents of Iloilo City.

  2. Attend orientation

A mandatory 2-hour session explains the rights, limits, and responsibilities of a health care proxy.

  3. Legal review & approval

The City Legal Office reviews the application, then the GAD Council Office grants final approval.

  4. Receive your QR ID

A PVC ID card with a QR code is issued. Hospitals scan it to confirm the proxy is valid — no login, any hour.

What you should know

You stay in control

The principal can revoke the proxy unilaterally and immediately. Revocation does not require returning the physical card.

Couples file reciprocally

A couple submits two linked designations — each naming the other. Either person can revoke their own without affecting the other's.

Privacy protected

Personal information is processed under RA 10173 (Data Privacy Act). Uploaded documents are encrypted at rest.

24/7 verification

Scanning a Right to Care ID opens the public verification page — available to any hospital, at any time, with no account.
